- Patient reference (avoid unnecessary personal details)
- Tooth numbers and restoration type
- Material, or the clinical situation if you want the laboratory’s recommendation
- Shade, plus stump shade for translucent materials
- Scan or impression of the preparation, the opposing arch and a bite registration
- Required date, and any notes on contacts, occlusion or embrasures
Shade communication
A photograph with the shade tab held in the same plane as the tooth, taken in neutral light, is more useful than a shade code alone. For translucent materials such as zirconia, the colour of the prepared tooth (stump shade) affects the final result.
Implant restorations
- Implant system, platform diameter and connection type
- Scan body used (digital) or impression coping and analogue (conventional)
- Screw-retained or cement-retained, and the abutment type wanted
- Soft-tissue information if the emergence profile is critical
Removable partial dentures
- Accurate impressions or scans of both arches, including the supporting tissues
- Prepared rest seats and guide planes, if your design requires them
- Jaw relation record and any design preferences (clasps, connectors)
Night guards
- Scans or impressions of both arches
- A bite registration at the desired opening
- Arch (upper or lower) and hard, soft or dual material preference
